Elderly Care
Our new Elderly Care Homes were opened in March 2018.
Elderly Care consists of three buildings which each 10 apartments. Our nurses provide guidance and help to elders who are not able to fully manage their day-to-day routines alone at home anymore. Each client has their own apartment including bathroom where they live semi-independent.
Elderly Care clients are persons who are semi-independent and still (partially) active and are 62 years or older. They must have an ADL (Activity of Daily Living) score of 6 or higher. Their independence has decreased and they are not able to continue to live on their own. The elderly clients are helped with their daily routines. Clients are encouraged to help themselves when they can and when needed are assisted by our nurses.
Admission Criteria
Documents required prior to admission for Elderly Care:

Potential client has at least reached pension age (currently 62 or older)

A score of 6 or higher for the Activities of Daily Living (this will be assessed by our team during the intake at home)

Copy of a valid health insurance

Copy of a SXM ID card or passport

A proof of registration from the Census office

Proof of pension from SZV and the willingness to submit this AOV pension to SZV as your own contribution towards the cost of care

Medical history from the family physician or medical specialist

A need for protected living with care and guidance (for details, see the other criteria below.

Other criteria (at least 5 of the 9 criteria need to apply):

(self)neglect

Abuse*

Lack of family care / family support

Lack of social independence

Difficulties in motoric functioning

Difficulties in psycho-social functioning

Difficulties in cognitive functioning (for example, onset of dementia, mild intellectual disability, brain trauma)

A need for assistance with personal care / self-care

Insufficient means of income to sustain oneself
*in the event the potential client is being abused, admission is possible without any of the other 8 criteria being applicable.
Follow Up
Once all documents are submitted, the client’s name will then be placed on the waiting list.
The social worker and department manager will then conduct a home visit, where the client’s needs are assessed and ADL score taken.
An AVBZ-care request is then filled in and submitted to the AVBZ Committee at SZV for approval. Once approved and when a space becomes available for the client, the family will be called and an admission date is planned.
